Terms and Conditions:

By participating in the LMA Golf Classic, you agree to the following:

1. Assumption of Risk: You acknowledge the inherent risks associated with golf and related activities, including any associated with the consumption of food and beverages. You voluntarily assume all risks, both known and unknown, associated with these activities.

2. Release of Liability: You hereby release, waive, discharge, and covenant not to sue Prevent CO Poisoning Project LLC, its affiliates, and all of their respective officers, agents, and volunteers (collectively, "Releasees") from any and all liabilities, claims, actions, damages, costs, or expenses of any nature whatsoever arising out of or in any way connected with your participation in the LMA Golf Classic, including those allegedly attributable to the negligent acts or omissions of the Releasees.

3. Alcohol and Food Consumption: You acknowledge that alcohol and food will be available at the event and take full responsibility for determining your legal and safe consumption limits and avoiding any foods to which you are allergic. You agree to not hold the Releasees liable for any injury, loss, or damage caused or claimed to be caused in whole or in part by your consumption of alcohol or any allergic reactions to food consumed at the event.

4. Fitness to Participate: You certify that you are physically fit and have not been otherwise informed by a physician. You are aware of your personal medical conditions and acknowledge that you should not participate in activities for which you are medically unfit.

5. Photography and Filming: You consent to being photographed and filmed during the event. You grant the LMA Golf Classic and its organizers the right to use your image, likeness, actions, and statements in any live or recorded audio, video, or photographic display or other transmission, exhibition, publication, or reproduction made of, or at, the event in any medium or context without further authorization or compensation.

6. Compliance with Rules: You agree to abide by all event rules and regulations established by the organizers, and you understand that failure to do so may result in your removal from the event without any refund.

I have read this agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my participation to be a complete and unconditional release of all liability to the greatest extent allowed by law.
